In which file are you using the custom icons? If you include the icons with the package thumb and can use it in the template.
Or if you editing any template file from the plugin, you can copy the template from plugin directory to activeTheme/download-manager/ folder then changes will not be changed.

You can show packages from certain categories with the all packages shortcode. But there is no option available for tags. You need to use the shortcode on any page to show the packages there. You can use TinyMCE button to easily insert the shortcode.

You can do customize design for wpdm-all-download by putting the wpdm-all-download.php file into the ActiveTheme/download-manager/ folder and modifying it.

Did you upload the pdf file or zipped it and uploaded the zip file? The default viewer with WordPress Download Manager Pro usage google doc viewer which only can preview public pdf documents. You can check this addon if you need any extra feature.

Need to check the file location. FIles should be in /uploads/download-manager-files folder. otherwise full URL to the file has to be defined in csv file.

Did you try to upload the sample file you given or please upload the original csv that you are trying to upload to check it. How many rows are there? Memory issue might happen when a file has too many rows.

1. You can deactivate and delete the existing plugin from Dashboard Plugins then download the latest one from purchase area and upload it through the plugins Add new. The old data will not be removed in this update.

2. You can use the email lock to capture the email while user downloads any package file. But it is not possible to disable it after the first download for certain time. You may look into membership features to make 6-month membership with a single email, and you can put packages under that membership level.
