Please check now we have resolved your issue and the download is working fine. There was a small bug in the form lock add-on. We have adjusted it on your site for now and it will be included in the next plugin update as well.
However, about the form lock check box, you have not entered the Terms and conditions checkbox labeling like this that’s why it seems wrong to you.

You don’t need to use the free version when you will use the Pro version. PHPMyAdmin is fine to use.
However, there is an alternative way. Simply you can just unzip it in your pc and upload it through FTP to plugins dir and then activate it from the plugins page.

There can be a conflict with another plugin, can you please enable/disable other plugins for checking, you may also switch the theme to another for checking, please let me know then.

Also, You can change the memory_limit in the php.ini for your server. If you don’t know how to change it then contact with hosting provider.
