However, about the form lock check box, you have not entered the Terms and conditions checkbox labeling

You don’t need to use the free version when you will use the Pro version. PHPMyAdmin is fine to use.
However, there is an alternative way. Simply you can just unzip it in your pc and upload it through FTP to plugins dir and then activate it from the plugins page.

There can be a conflict with another plugin, can you please enable/disable other plugins for checking, you may also switch the theme to another for checking, please let me know then.
Also, You can change the memory_limit
in the php.ini
for your server. If you don’t know how to change it then contact with hosting provider.
