the button should show a popup and show the short-code. However, you can generate shortcode easily ( rather than copy/paste it ) using any of the following add-ons:

If you are using the classic editor:

Advanced TinyMCE button

For Gutenberg users:

Gutenberg Blocks

And for elementor:

Elementor Addons

Also, such an issue only will happen if any other active plugin and theme style conflicting on that page, you may try deactivating other plugins to find the conflicting one.

On the other ticket, you said you are using Formidable and Contact Form 7. I think those plugins are conflicting with the form lock somewhere, can you please enable/disable those plugins and other plugins for checking, you may also switch theme to another for checking, please let me know then.

you can try and follow the link below documentation,

Template Files

by default, you are using link-template-default.php for link template and page-template-default.php for the page template. As documentation, you have to copy the file in the theme directory naming a folder called download-manager, then paste it in page-templates for page template or link-templates for link template. And you have to replace [download_link] with <a href="[download_url]" class="btn btn-primary" target="_blank">[link_label]</a> in related PHP file.

Add the following code after line 235 in /download-manager/tpls/wpdm-all-downloads.php file,
