Purchased Form Lock to add to Free Version – does not work

I installed the Form Lock on the free version. Everything at first glance seems to work. However, once you go to actually download the file, it hangs up on the loading gif and the form never actually shows.

All other locks work just fine but Form Lock causes it to hang up just like this.

On the other ticket, you said you are using Formidable and Contact Form 7. I think those plugins are conflicting with the form lock somewhere, can you please enable/disable those plugins and other plugins for checking, you may also switch theme to another for checking, please let me know then.

I changed the theme to 2020 and 2019 and both produce the same result.

I disabled both Formidable and Contact Form 7, and both produce the same result.
