which link-templates you are using in that shortcode, you can customize the link-template, you can follow this doc, https://www.wpdownloadmanager.com/building-a-wordpress-download-manager-link-template-from-scratch/ also you can edit the link template PHP file,

and for media image as an icon, you can use like as I image attached

I have checked that category Access role is empty selected, can you please elaborate more on your issue?

I have seen that Allow Access option for the blocked packages is empty, however, when I give All Visitors role to the package, it gets disappear, also disappear retrieving the empty option again, can you please update your plugin to the latest version, it is also an old version now,
