The block you are using to display the packages only supports Parent categories. This is why content under a Parent category does not appear if it has no Child categories.
I would suggest you to use WPDM Elementor add-on, where you can use Packages by Category block and include both child and parent categories as per your requirement.

I have checked the form using Contact Form 7 in our test setup, and it is working fine. This issue may be caused by a plugin conflict. May I check by deactivating all plugins except Download Manager, Form Lock, and Contact Form 7?

The 4.5.0 version you’re seeing was part of an older Pro edition of the Download Manager plugin, the current pro version is 7.1.0. V3.3.46 is the latest free version.
Here is the Download Manager Pro changelog.
Since this is a very old version of Download Manager, it’s difficult to guarantee that a migration would work smoothly or without issues.

The error was related to v3.3.38 and was immediately resolved. As you can see , the date mentioned in the error is 23 December.
