A new Category is not showing. It has the same settings as all other categories, and it has one file in it. It has no Parent Category.
If I use a Parent Category, it shows.
It is not possible to have a Category with files, without having a Subcategory?

Are you referring to this issue,
A top-level(parent) category with files doesn’t show
A subcategory(child) with the same files does show

Yes, that is the issue.
Kindly share temporary login credentials and related URL in a private reply to check the issue.
The block you are using to display the packages only supports Parent categories. This is why content under a Parent category does not appear if it has no Child categories.
I would suggest you to use WPDM Elementor add-on, where you can use Packages by Category block and include both child and parent categories as per your requirement.
