Sorry to hear about the issue.

What I think that You are using old codes in your currently activated theme. That’s why it’s showing such errors.
I would suggest you to update wpdm to the latest version 6.1.5 and update all the addons of wpdm if you are using; to the latest version.

However, it will be very cooperative if you record reproducing your issue using a tool like this?

Please make sure you are using the latest version of wpdm that is 6.1.5
If you using this, you should be able to see some pre-built templates like below:

In the link template list, the template you need is the Link Group which template ID is link-template-bsthumnail

Just use the template id for the template parameter in shortcode.

I am sorry that there is issue in the CSV import category feature with the current version of WPDM, it needs to be fixed in the upcoming update to work properly.
However, as the files are being uploaded properly, I think you can use the quick edit option to assign proper categories.

I have replace wpdm with the 6.1.4 version. You can try that for importing.
After adding the downloads, let me know where you would like to order the downloads. Then it will be understandable to you clearly.

1. I am sorry as I checked the issue in more details and found that it is a new issue in the version wpdm 6.1.5 . However, it was working properly in the version 6.1.4. I hope soon it will be fixed in the next updated version.

2. Ordering is a different issue. You can manage ordering after uploading the downloads through shortcodes parameter or in other ways in the page you will show the downloads.

a) You can disable the single file download button by following below settings in your site from Downloads > Settings > Basic

b) The title is getting from theme single.php file. You have to modify that to change the grey title.
c) You can disable comments by following this

1. In our test environment, category and tags working properly. You can test here
However, I am not sure about the issue. There could be conflict because of activated theme or any other plugins. Or there could be permission issue in your server. I would suggest to test all manually.
2. I am not sure what is the character that. I just used the csv you shared and the character is in that. You can just edit the csv to change the character.
3. No. You must upload the files to ‘download-manager-files’ directory to get those dynamically after uploading the csv.
