You have to use a link template that will be used in that shortcode. In the link template you have to make sure the download button has the direct download link template tag. For link template and templates tags you can follow this doc.

Here you can check the available built in link templates also you can use them or modify them to use.

It is a javascript issue. sometimes cache plugin causes the issue. In this case you can clear cache or disable it for sometimes to check if the issue persists.

After doing so, If the issue persists, please also share temporary admin access in a private reply to check further.

You can use custom css to increase button size using css like below in Appearance > Customize > Additional CSS

In our development environment, form lock is working properly without any error.

First of all please make sure you are using minimum version of 7.4 of php to run your website.
Then You need to run a conflict test on your site to detect what actually causes the problem. if there are any conflicts among the installed plugins. To troubleshoot the issue, please deactivate other plugins except for WPDM with all addons of WPDM if you are using and check if the issue still persists. If that solves your problem, activate the plugins one by one, and continue checking the issue. In this way, you will be able to detect which one causes the problem.

To get direct download links, please upload files using select from media library this feature .
However, remember it is default WordPress upload feature and wpdm has no control over it.
