There were few forms without entries. They were also tested and entries were added.
So I think there is no issue.

I found that the issue is persisting because of conflict with any other plugin.
I tried import that sample file after deactivating all other plugins except WPDM and found that it is working properly.
So I would suggest you to follow this way.
Also if you want to know the exact plugin which is causing the conflict, then you can disable and enable plugins one by one and test csv upload to identify the culprit. That is time consuming so I would suggest to just follow the first way and import your csv.

Sometimes server doesn’t allow plugin updates directly for permission issue. In this case please download and upload to your website plugins from here.

Without coding also You can control the view from “Category Page Template”
If still you think this doesn’t solve your issue, please provide me related urls and temporary admin access of your website in a private reply also please record the issue using tool like this to understand it properly and solve immediately.

The error message is not related to wpdm.
It is coming from the mentioned plugin ../includes/message.php
file.
So to change that you have to change or modify that plugin.

Adding media files are better for images or videos. That is the default wordpress uploader. It is always better to use the upload system of wpdm. In that case, wpdm can affect some features of the uploaded file. That fixes many issues.
Yes with that shortcode you can add any category you want. For that you have to use the parameter categories
in that category slug.
Example:
[wpdm_all_packages items_per_page="20" categories="slug1,slug2,..." jstable=1 order_by="field_name" order="ASC/DESC" cols="title,file_count,download_count|categories|update_date|download_link" colheads="Title|Categories|Update Date|Download::155px"]
And yes search field, search only in the current category you are showing.

Please update wpdm to the latest version 6.1.9 as well as other wpdm plugins. It will solve few issues and enable new feature of wpdm.
Actually there is the text for user level. But somehow it is overridden by Avada css that opacity should be 0. I found the css is being get from here: https://www.rzfun.com/wp-content/uploads/fusion-styles/d0d2d8502f5f8d3da028567380ff55ed.min.css?ver=3.7 . Getting this opacity from the class fade
. setting the opacity:inherit
will fix the issue.
That is the reason it is not showing.
