I think for form lock contact form 7 is somewhere is conflicting, can you please enable/disable other plugins for checking, you may also switch theme to another for checking, please let me know then

For different file type, for different icons, and for the bulk apply there may require special customization or code adjustments. or you can keep the default icon for the default file type as usual

The page template was missing the tags tag for the default template, for tags showing it needs the tag for the default template, please check

you can use the media library image, you can copy the URL of the media library image/icon and use it in the Icons section as a URL, please check and let me know if it works for you
