It maybe conflict with htaccess role or can also happen due to other plugin scripts conflicts. It may have between your plugins or theme. can you please enable/disable other plugins for checking, you may also switch theme to another for checking. Is there any additional section or popup appearing from your side?
For #3, For download finishing handing indicator is related to browser, when you hit a download button download finish process handled by browser. I hope you have understood.
