actually there is no special setup option there, just install and activate like regular theme, that’s all, from theme option you can change some other settings and which are explained there.

user don’t have to use default login option, you can activate custom login form from download manager settings page which will be shown in page details page when user is not logged in, after login there he will see the download link. Everything will be in same page.
