Sorry for the unexpected delay.
“I can’t get the login to route members to the pages created for each membership type” — nor clear, however you only need single login page here for all membership levels. May you please give me some example paged you have created for different membership levels.
For now, Using wp pro membership plugin you can assign single membership level to an user, but we will add multiple membership level per user with the next update of the add-on.
Sometime issue are server specific and there is nothing much to explain, but I’m not sure what exactly you talking about. Give me temporary wp-admin login info in a private reply to check your issue.

sorry, not clear, do you want to attach image as feature image or downloadable file.
For now, From front-end any user only will see the files created by himself.
You need to download latest version from here http://www.wpdownloadmanager.com/purchases/ , then deactivate and delete old version , now install and activate new version. FYI, deleting old version will not delete your data.
