You need to install wpdmpp-2co.zip to add 2checkout, then you will be able to configure it form Admin Menu Downloads Settings Premium Packages Payment Settings Tab.

Please get the attach file ( functions.php ) and upload it in /plugins/download-manager/ dir, issue is fixed there.

FYI: from v4.4, you can use category name or id with csv file, if category already doesn’t exists it will be created automatically.
