Short-code are showing now, there was a file version mismatch. But now download link is not working as files are not in /uploads/download-manager-files dir, please let me know where did you kept the files.
If you already send the package, they already have it, blocking email will block them, when they try to download new item by entering their blocked email.
Email lock is a totally functional feature ( Demo: https://demo.wpdownloadmanager.com/wpdmpro/download/password-locked/ ), nothing to change in code for that. Also, checked code in your site, the mail function is executing, it is something around your server.

That is happening for PHP version incompatibility. Please download wpdm pro again from here https://www.wpdownloadmanager.com/user-dashboard/purchases/ and update. That will fix the issue.
In that can you can create a package with no file attached or a file with the message like “Please wait for review”, after your complete purchase, if you approve it, you can attach the download with his order using order note option.
