Sorry for the inconvenience.
Please update the WPDM plugin to the latest version which was released yesterday.Also update all the addons of WPDM you are using.
From the version 6, codebase is changed for WPDM. So it is must.

After that, As template and shortcode structure have been updated, Please update the related download packages/page again by selecting and changing link template/page template. It will fix the issue I hope.

Make sure that WPDM and all add-ons you are using of WPDM are in the latest version. We released new version of WPDM yesterday. so please update your WPDM version too. Also for page template, Please update from here.

The problem is here. You have used all header in same column.

This should be arranged like this:

If you see the sample, you wil find that individual column needs individual header/field name. You csv file should be organized like that.

You can always download update from here. So you will not have any issue in live website.

I have checked it.
Email is working correctly. The problem is the mail is being stored in spam folder
That issue is not related with WPDM. Please make sure that carrier email doesn’t mark your mails as suspecious.
