Use ID Heading, For an exiting package, for the cases, you are updating exciting packages. And also use the preview heading in the CSV file for a Featured image. and please follow this CSV file structure,

For invoices there needs billing info, you have to enable the billing address on the checkout page. So this warning is showing. Thus the invoice is created. I hope you have understood

For #1 and #2 issues, I have found the issue, soon you shall be updated
and for #3,
there is a typo mistake, you can check it from
wp-content\plugins\wpdm-premium-packages\templates\partials\guest-order-details.php file line number 67, there text domain is not same, you may also can update it from yourself. please check
