Please keep such this way wp-content/themes/ACTIVE_THEME/download-manager/all-packages-shortcode.php for overwriting the file from your active theme, I hope it should work properly then. And for admin menu issue, please deactivate > Delete and reinstall the WPDM plugin again, I hope it will fix the issue then. Please kindly check.

You can disable one by one other plugins for testing if there any conflicts. you may also switch theme to another for checking.
