From Quick Forms menu everything is coming properly from my side as I image attached,
However with Create Form the form is not coming. I think for this, it may have a conflict between your plugins or theme. can you please enable/disable other plugins for checking, you may also switch theme to another for checking. Please kindly check and let me know then.

For now, User Dashboard > My Downloads section search option shall not work with keyword of package description, however, you can also keep a feature request here, https://www.wpdownloadmanager.com/support/forum/download-manager-pro-feature-request/. Please go to Downloads > Templates section, then you can clone the Call to Action 4 template , from template edit option and then editor remove [download_count] template tag, then save the template and use this template in your package settings. Please check which template you are using from your package settings and go to Downloads > Templates section, then you can create a new template or also able to clone the existing templates , from template editor remove [download_count] template tag. For These packages are for any user who is logged in, to be able to download for enabling this you have to set Allow Access option for restricting.
