Seems like you have set all visitor to all of your packages. If you want to only make the file download to the logged in members, then it needs to be put subscriber in the allow access. Can I do that and set up and the download manager login/reg page?

You can set Billing Cycle Limit to bill only limited time after the membership time expired. But you need to use download limit add-on to limit download for specific membership level.

Try to download the plugin from the purchase area and delete the old one and install the new one from plugins. Options should not be disappeared.

You can use this filter to add a menu in the dashboard.

As you are using the free version of the plugin. You need to edit the template file by modifying the template file from /plugins/download-manager/tpls/page-template/.
