Looks like main preview image is missing from that package

may be your cache dir ( /plugins/download-manager/cache/ ) is not writable. Please make cache dir writable, if that doesn’t resolve your issue please mail wp-admin login info to support@wpdownloadmanager.com

It is strange. please send a temporary wp-admin login info to support@wpdownloadmanager.com to check your issues.

1. it happening because your theme css conflicts with bootstrap.css, can be resolve just delete some css from bootstrap.css
2. delete the file: /plugins/download-manager/modules/css.php . not required anymore from WPDM Pro Xtreme ( v3.3.3 )
3. select default link template or user extra class with download link tag, for example:

1. [download_link btn] or
2. [download_link btn btn-large] or
3. [download_link btn btn-large btn-success]

here btn-lage for large button

btn-sccess for green button

similarly you can try , btn-info, btn-inverse, btn-danger , full button class reference is here http://twitter.github.com/bootstrap/base-css.html#buttons

for that you simply need to modify line# 1010 at functions.php,
replace:
