You need to use absolute file path for preview image, not URL.

1. you can use custom pricing table plugin for creating membership plans page and copy plan link in pricing table action button link to custom design or you can use some custom css to customize current design.

the category drop down will work fine, you need to disable bootstrap js from wpdm settings, as bootstrap js already included by your theme.

Login info is not working.

ERROR: Invalid username.

It may happen if you installed any cache plugin, you need to exclude download link ( \?wpdmdl.* ) from page cache.
