Are you trying to get alt text while uploading through media library, you can get like this,

$image_alt = get_post_meta($image_id, '_wp_attachment_image_alt', true);

What type of behaviour is creating when you are pressing the download button?

It can be a browser cache issue, but the behaviour will be the same.
