Yes, one user needs to input email in the locked package. like, when user will enter their email address to download the package the email automatically will be added to your selected MailChimp list.

There are many issues in your CSV file, like the icon, preview column values. please check the https://www.wpdownloadmanager.com/doc/bulk-import/ for the column values or follow the sample.csv file.
