If you are using a very old version of WPDM, there may some functions not work properly, so you should use the latest version, unless for the old version you may mail support@wpdownloadmanager.com for the old version

You shall get the option from Downloads > Settings > Quick Forms as I image attached,

also, you shall get an option in Downloads > Custom Fields

Sending Email to certain mail addresses, there should set the mail with email template or any other adjusted plugins, otherwise, email should not be sent for multiple emails, please check your other plugins settings
