Sorry for the inconvenience. It is working fine from my side. I think it may have a conflict between your plugins or theme. can you please enable/disable other plugins for checking, you may also switch the theme to another for checking. if possible, please, give your temporary wp-admin login details in a private reply for checking the issue.

Yes, For form-lock it will not show the File URL. For filling up the form, it will give you a popup form. Thus the download for the file will finish. I hope you have understood
