Hope you are well. please update your WPDM plugin to the latest version. I hope it will work fine then. For updating the WPDM PRO main plugin, Please simply click the Save Settings again for the license key option from Downloads > Settings > License, and then please update the plugin from the wp-admin > plugins panel.
Alternatively, You can also download them from here.

If you are trying to implement a parent category browse to the sub-categories, and all of the categories should contain category images and they should be for an individual page. Please create/clone a page-template from any existing page-template from your this directory,
wp-content\plugins\download-manager\src\Package\views\page-templates
please also change/adjust this portion too from your page template

and use this code snippet anywhere in your PHP page template,

The download with Form lock + gravity form is working properly now. I have tested it here now,

The issue was you were using an old version of WPDM. I have seen that you have an active license key. For updating the WPDM main plugin, Please simply click the Save Settings again for the license key option from Downloads > Settings > License, and then please update the plugin from the wp-admin > plugins panel.
