In my inspection, I found you are using an older version of the WPDM Pro and the Advanced Access Control plugin.
We have released the latest version 6.0.7
of the WPDM PRO plugin and add-ons. According to the latest update of WPDM PRO, you also need to update the other add-ons as well. Please update them specially the Advanced Access Control add-on, or you can download them from here.
I hope it will work fine then. Auto-install sometimes fails due to auth error or server file permission issue, can you please try after manually install the plugins?

Can you please uninstall and then remove the older version of the WPDM Pro plugin first.
After that reinstall the latest version again.

2. Please follow this screenshot in order to edit the relevant PHP file.
3. Not clear about this. Will you please explain a bit more by providing some screenshots or even a screencast?
4. Simply just assign a role for the specific category and make sure that you have removed the assigned user role from the package settings. It should work.
5. Currently, this is not possible. However, you can request it as a feature here. You can use Edit Package from the admin bar as well.

When you will not select an icon WPDM automatically assign a file icon based on the attached file type, for some shortcodes. And that icon is SVG one. So by selecting the png icon when creating a package, the png icons will be applied always. Please check this.

Yes, exactly whenever you made a zip in MACOSX it will add _MACOSX which will not show in MACOSX but however it will show in the windows or other OS platforms because _MACOSX is not supported on them.
