actually most of those problem you facing is just administration issue, those are not bug with plugin. you simply need to handle the thing properly.

If icons are missing or showing broken, then its something with your system. as if you check our demo, all icons are showing properly, also working fine for other users.

now you can check with your hosting/server support, its not a problem related with wpdm pro.

Showing now. You will not see it in template link, but you can edit it from plugin editor ( filename is: /download-manager/templates/link-template-widget.php )

1. Download latest version of WordPress Download Manager Pro from same location in orders page.
2. Unzip the package
3. Upload files to /wp-content/plugins/download-manager/ using ftp
4. login to wp admin
5. deactivate download manager
6. re-activate download manager
7. Download and install copy to pro add-on from here http://www.wpdownloadmanager.com/download/copy-to-pro/
8. Go to wp-admin Downloads Settings Copy to premium tab, and you will see the list of files you create with free version, check all copy
9. and done!

it will be fixed with next update, coming on 5th Nov.
