“link/page template are not working fine” <-- reupdate your plugin file, it could happen if all files not updated properly. Most probably category mess up also happened for same reason, as i just checked in code, importing category don't remove old categories, import action just add new categories after exiting ones. Anyhow in next update we are upgrading import section for few points.

recreate those with same name, or create the csv import file for all cats , then import it. But don’t need to reassign if you keep the names same

looks like your are using any plugin which is using simple-modal js library. That may cause a css conflict, please send a temporary wp-admin login info to support@wpdownloadmanager.com to check and adjust your issue.
