actually I’m not understanding how the error is coming, as in my side it looks ok and at line #604 , no code to show such error. but please send a temporary wp-admin login info to support@wpdownloadmanager.com , support team will check this.
if you have problem with ftp, then simply delete old version from wp-admin and re-install new version.

thumbnail preview for individual file, not exactly sure about the implementation of that feature as not so many people asked for that :).
but you still can submit it in feature request. Add-on coming within this month.
1) because, your category name is too login , try using full width page template there
2) same reason as #1
