should be ok now. you were using old version ( 1.0.0 ) of custom field add-on, current version is 1.1.0 and I’ve updated it there.
not for a package it is already created, but it simply will keep some option/value auto-selected before your create the package, so that you don’t have to reselect everytime when you create a new package. anyhow give me login info again in a private reply, I’ll check.

looks like you have network activate the plugin. deactivate the plugin from network admin and activate it from subsite.

default value add-on only auto select some default values when you creating new package.
