it will depend on your theme, need to remove author tag in case of wpdm pro.
from page template select the template with sidebar, it is simply as you do for regular wp page, but totally will depend on your theme.
[download_url] will not work for locked package, if you apply lock option you must have to use [download_link] which generates full html for download link including all required lock option.

looks like you have network activate the plugin. deactivate the plugin from network admin and activate it from subsite.

if you delete actually that is not our problem. if you think that is not the way you wanted it, though it was working fine, we offered to redo it. And no matter from where we are and $15/h, that’s not us, it was better for you to try someone first with $15/h. About the quality we think, it’s perfect as long as it meets all requirement where you didn’t give any specific mockup. Now you are coming with all unnecessary arguments, if you could find some better for free, why did you ask for customization. But still we don’t know how you want it. But we have backup and can prove that everything is working as per your job description. but if you have better idea, you could tell us to do it in that way. And our customization team do more then 400 hours of csutom upgrade work on wpdm pro every month in average, so they know, how to do things. If you don’t like the way it was done, tell us your way, we will redo it just as a special offer though it is total waste of our time.
Latest version don’t have company and title field actually and there are no easy way to add those actually.

to show csv imported custom fields there will require some coding, as by default it supports only specific fields with wpdm pro.
