I see there are lots of plugins installed in your site, I tested disabling other plugins and category page worked fine, you can find the conflicting one by disabling one after another 🙂

wpdm adds such prefix when another file with same name already exist, but it is trimmed before download, however you also can choose option to overwrite files with same name to avoid such rename from wpdm settings page.
I see download button also showing properly at front-end now, addtionally, I installed image button add-on ( http://www.wpdownloadmanager.com/download/wpdm-image-button/ ) to show nice images with download link. You can change button image from wpdm settings page.
it will not be able to stamp file in zip, you need to enable single file download.
in your log, it showing /public_html/wp-content/plugins/download-manager.new/templates but no such dir there actually, it is /public_html/wp-content/plugins/download-manager/templates and site loods fast enough from my location and download manage is active.
You can enabled email lock from Package Settings Lock Options and check ask for name option there, at front-end user need to enter name and email to download the item, when you are using mailchimp add-on, the name and will be added with selected mail list automatically, but there are no further option to change labels or field names. However, please let me know know if you need help with activating email lock.

It may happen if loop in archive page template is not coded properly but it is hard to say exactly where in side the theme without checking.
in wpdm-functions.php, replaced post_content' ► $drow['description'], with post_content' ► utf8_encode($drow['description']),
the ftp account is pointed to a blank dir, may you please point it to plugins dir.
link-template-calltoaction2.php file originally have only few lines of template tags and that even shouldn’t take any mentionable execution time, however if you use any cache plugin like wp super cache it certainly will resolve any such issue, but I want to check the file in your installation to check if that is changed somehow. The demo is using this theme http://www.wpdownloadmanager.com/download/crypton/ , however, you can create similar blocks using category short-code and link template panel even without that theme.
The problem is actually with Divi theme, any function or module in divi causing conflict with bootstrap accordion, may you please ask divi support to check the issue.

Still you were having problem because the description in csv file has invalid utf8 chars, anyhow it will work fine now, as I applied utf8_encode function for description before inserting post.

Sorry, no such option in wpdm pro. Probably you can do something like that using yoast seo plugin, but that functionality is not within the scope of wpdm pro 🙂 .
Got it, may you please give me ftp access to plugins dir, I tried to update a file from wp-admin and it didn’t work.
