Hope you are well. If you are trying to add a category title on the WPDM category page, you can use the default category option from the edit category page, Do Not Apply option, thus it will use the theme archive page, or you can also use taxonomy-wpdmcategory.php from your theme too.

And for breadcrumbs, you can use the single-wpdmpro.php file copying the single.php file from your theme. And you can use there your own customized breadcrumbs appearance. For without plugin breadcrumbs functionality you can check this article, https://gist.github.com/tinotriste/5387124

As per rules, Package Access = Package Access(subscriber) + Category Access(Subscribers) = Subscribers should watch the package, you have choose multiple roles from category, so it can be access from various roles. I hope you have understood.
