For your first issue, the Download History section is responsible for the packages and files which you have downloaded recently.
For the second, the My Downloads section is responsible for the packages shared via the members which you have been set from Package Settings > Select Member section. please check, I hope you have understood now.

The CSV upload for the packages, you have shared CSV file is working fine from my side except Sorry! File not found! issue. For this issue, At first please upload and keep the files with the same title in the wp-content/uploads/download-manager-files directory. And for the ampersand character issue, for now, please press the update button again for the packages that causing the issue. Thus, I hope it should work. please try and let me know then.

You have used template=”default”, which means you are using, the link-template-default link template. you can duplicate the link template from downloads > settings > Template > link template and adjust that with the above [title] template tag. I hope your requirement will work fine then. please check and let me know.
