To appear button style exactly you need to activate css file from wp-admin Downloads Settings Front-end css tab .
but what is the problem with category embed code?

but if you use tinymce button add-on ( free ) here http://www.wpdownloadmanager.com/download/advanced-tinymce-button/ , it will be more easy

now you can check with your hosting/server support, its not a problem related with wpdm pro.

1. Download latest version of WordPress Download Manager Pro from same location in orders page.
2. Unzip the package
3. Upload files to /wp-content/plugins/download-manager/ using ftp
4. login to wp admin
5. deactivate download manager
6. re-activate download manager
7. Download and install copy to pro add-on from here http://www.wpdownloadmanager.com/download/copy-to-pro/
8. Go to wp-admin Downloads Settings Copy to premium tab, and you will see the list of files you create with free version, check all copy
9. and done!

no, but you can select link template for category while creating/editing the category
