I see you don’t have this http://www.wpdownloadmanager.com/download/premium-package/ add-on active, you will need the add-on for that theme along with wpdm pro. We mentioned it here http://www.wpdownloadmanager.com/download/marketplace/ in description.
#1) frontend was not showing as you didn’t setup permission from Downloads Settings Front-end Tab , anyhow I set it up there now.
