From settings page set remote ftp info and save, then from add/edit package window you can browse files from remote ftp and attach with your package.
go to edit package window and select the page template from page settings as per your need 🙂
double slash will not cuase any issue, anyhow please make cache dir writable, that will resolve your issue.

Those are simply because of your theme css conflict. Need to adjust theme css.

looks like all files were not uploaded properly, try delete current version and install new one.
You don’t have to delete any category, I don’t see any issue with category assignment there, also the category assignment is totally core part of wp, not even coming from plugin. May you please take a short-code screen record of the issue, so that I can see how it is generating.
You can’t allow unregistered user to access that page for creating package. Only allowed logged in user can have access to that page and create package from front-end.
