Actually looks and feel for category page will depends on your theme or need to develop custom page template for showing categories as per your requirement, anyhow we have custom upgrade service ( http://www.wpdownloadmanager.com/download/custom-upgrade-service/ ) if you need custom theme/template development like that :).

2 ways:
#1. You can use category short-code ( [wpdm_category] ) in a page, use this add-on http://www.wpdownloadmanager.com/download/advanced-tinymce-button/ to generate short-code

#2. create custom page template in your theme for download manager categories.

ok now, the issue was because the cache dir took all space in your hosting. Either upload zipped file or clear cache dir sometimes if you see any such issue. Usually cache dir is truncated in few hours interval.

Yes, that was from earlier version, v4 is not using it anymore. You can delete the table.

the feature still experimental and released with limited option. Need to activate “Individual File Download” for document preview. It is not a bug. If you use [file_list] tag with link/page template, when “Individual File Download” activate, you will see download button next to each file title.
