@Hansatech, old data or settings will not cause any issue, just deactivate & delete current installation , then install and activate the plugin again, everything should work fine after that.

As it was working before, it is certainly something with your server, updating wp will not cause any such issue. May you please ask your server/hosting support.

Did you delete “All Visitors”, if still “All Visitors” is there, then everyone will be able to see and download the file ? However if that is not the case, Probably all visitors is selected for assigned category. You need to uncheck all visitors from any assigned category too.

Not sure exactly, but issue in not form wpdm, Please give me temporary wp-admin login info in a private reply to check it in your installation. BTW, you may try disabling any recently installed plugins.
