I see you are using old version of WordPress Download Manager Pro, please update your copy. That will fix the issue.

Template preview use first package from your package list to create preview. You were not seeing any template preview as your first package was assigned to “Subscriber” role only, means, you ( administrator) don’t have access to that package and in wpdm settings Hide Everything was selected for the option When user is not allowed to download:, so in preview it actually hiding everything. Now I’ve select “Administrator” role too for access field and preview is showing fine.
