You need to customize the code directly to get this. You can follow the steps to customize:
You need to copy thetpls dir from download manager plugin dir to active theme dir.
Them rename it copied tpls dir ( which is in your theme dir ) to download-manager
Now edit wpdm-all-downloads.php in download-manager dir ( which is in your active theme dir )
Maybe update was not completed. Extremely sorry for your inconvenience. Please try deactivating and installing the plugin again.
This problem may be due to theme. Please try switching the theme of your site. If your problem is not solved please provide me temp login info to check the issue.

To disable you need to check the check boxes and save settings. Unchecked means Enabled/Active.
Usually pop-up issue is due to bootstrap present in your theme which may conflict bootstrap in wpdm. Try disabling Bootstrap js with Downloads->Settings->Basic->Misc Settings.

Its locked at the top already. May be installation was not completed correctly. Or may be its conflicting with other plugins..

I can see you dint active “WPDM – Premium Packages” plugin. I did that now.
You will get a option to add settings.
Go to wp-admin->Download->settings-> Premium Package-> Payment Options -> Paypal Pro (active and set your credentials)-> save
This is nothing to do with download-manager plugin.
You need to change in your theme. Please check the official docs here: https://codex.wordpress.org/Designing_Headers

You misunderstood the plugin functionality. This plugin is used to change download button look only.
