For [wpdm_frontend] shortcode responsible with author-dashboard.php this PHP file from tpls folder. you should follow the same rules for creating and saving the edited file.

It needs some customization. Please, give your temporary wp-admin login details in a private reply for adjusting. i shall adjust it for you. otherwise, you can also adjust it, Copy wpdm-dashboard.php from download-manager/tpls folder and paste it in ACTIVE_THEME/download-manager/wpdm-dashboard.php like this. and adjust the file wpdm-dashboard.php as your own requirements.

Please delete and install again WPDM. I think icons will be appeared again if the luck favours or any misconfiguration there. let me know.
