Really sorry for the inconvenience. However, it was just basic settings setup from Downloads > Settings > User Interface > Changing Bootstrap 3 to 4 change. Now everything is Ok. Additionally, I have to tell you that, I was set the premium package settings in the basic form. please set it now as your requirements. and let me know if need any help

Hidden field and this tag [wpdm_package_id] as a default value, the tag will be replaced by package ID automatically when rendering a form only works with Gravity Form.
