I have adjusted a line code plugins/wpdm-form-lock/wpdm-form-lock.php in line number 150 with this,

$formhtml = str_replace("[wpdm_package_title]", get_the_title($pid), $formhtml);

now when you will use [wpdm_package_title] in gravity form, it will capture the package title in entities. However, it is not integrated into our server add-on, but it has been adjusted on your side. I have tested here, https://staging-susaguhlrealestate.temp312.kinsta.cloud/download/wpdm-test-2/, it is working fine. please check

They are saying that,

* 34 requests returned HTTP 500, which is an indicator of a server error on your end.
* 1 request timed out, make sure your server is able to respond fast enough and that you do as much work asynchronously as possible.

It is your server related problem. I think it is better to contact your server administrator.
