we have an add-on here http://www.wpdownloadmanager.com/download/user-review-add-on/ for rate & review, bit download condition can’t be applied with this be default, anyhow can be achieved through some customization, also microform add-on add form lock option but the add-on still don’t have rating option.

actually for first time while creating package it is setting to 0. LOL, I never guessed you would want to add a count even when creating a package.
if they are already in server, simply get the url and use it in remote url field while creating a package.
