#1. Previous/Next is coming from your theme, please ask theme developer to remove those.

in line #15, we used session_start function, session is required for password protected downloads and 99.999% server/hosting are session enabled. Error in line #15 means, your server disabled session_start function. They simply should enable it to resolve this issue.

Custom roles should show automatically in that list.
On wrong password you should see error message except there are some js conflict, please give me your URL to check

This is because your server is blocking “http://vancouverps.staging.wpengine.com/wp-content/plugins/download-manager/js/jquery.cookie.js” please ask server support to unblock it ( make it readable from http )
