As i mentioned earlier, this issue is occurring due to other plugins which is adding few more columns in the table. You can simply select the Columns you want to show in the table.
You can also show / hide this new column by opening the Screen Options panel and clicking the checkbox. Please check the image attached.

Login and registration tab was bootstrap default js is been overriding by your themejs.
I added some code in bottom of download-manager/tpls/wpdm-be-member.php file

If the user is not register it do shows error prompt.

Try using a short-code like this:

[wpdm_login_form redirect="url-here"]

then, user will be redirect to mentioned URL.

Did you mean each file unique code to download?
To do so you need to Enable Password Lock option. You can enable it from Package Settings Lock options tab and set a package password there, then individual file password will work.
wp-admin-> downloads -> Select file -> Package settings -> Lock options ->Enable Password Lock-> enter password -> update
